I have noticed the same thing with the city polygon changing shape, appearing and disappearing, etc. I wonder if it is similar to very large landmarks -- those don't display their full shape unless there is a road with in each tile, which can be rare for large national forests. So maybe these ghost polygons are connected to tiles that either don't have any roads or just don't have roads which have been updated recently.

You are right: even if you rename the streets correctly to split the cities or to move them to their correct location, the wrong name remains forever in the client tiles where nothing has changed. This may be inside a city where the roads have not been edited for some time or in the large areas without the roads. Also note that the outdated areas may be different at different zoom levels. The "Map Editors" color scheme will display not only the wrong names but also the wrong areas.
