It looks like a level 1 editor deleted the on-ramp to Rte 128 and then tried to re-create it here, but of course could not connect it to the highway. I suspect this accounts for the extraordinary collection of URs around this interchange. 
Also, the turn arrows on two nearby segments here and here appear to be wrong.
