Looking for consensus regarding these 2 points:
2 - If ramp type is used the road name should NEVER be blank.

Unless that ramp section connects to multiple exits. This would be one of the only cases that a ramp segment would be left nameless.

example: A ramp leads to another highway, and two different streets (Multiple BGSs)

I was literally typing this out when I saw you posted. Thank you for pointing this out.

#2- Would change as follows:
Usage of the ramp type, per the definition of the guidelines, would require an exit name on at least one segment of the jughandle.
