Ah, U R right, I didn't notice that.
Yes adding dogleg on incoming & outgoing segment will fix the case I tried it in other samples & it worked, I just want to try other solutions, so I'll know all solutions for different cases.
I added a node to median segment to try this solution just to get sure this works also. :)

Each of them looks good. Most of the following is just me talking to talk

I generally dislike adding a node in the middle as it can corrupt turn timings.

With that said, adding another node in the middle of the segment causes the rule to recalculate. In that example, you will have a 90 degree incoming angle and a 0 degree outgoing angle, so the rule is still in place, however, it is only applied to a segment with its incoming and outgoing angles, or in other words, three segments.

Using the doglegs is often how to control for for divided roads which do have a left turn attached.

So for this example - let us say that only the West back to East U-Turn is allowed, but the East back to West is not. if you set the segment to be under 15m, you can put in a dogleg to allow the U-Turn in one direction but not the other.

I see that regularly here in the United States.

Because using turn restrictions will prevent left turns through the median segment, there is not another way to control a U-Turn like that for a divided highway unless you use a Bowtie Junction.

(By the way, going around with shift + Z activated I noticed that these turns are restricted in all directions - is that intentional?)

Again each of the examples look good! Please let us know if you have any questions!

Finally INTL map tiles updated after long delay.
Adding node in median segment also solved the problem.
It was just a test to be sure this solution also works.

The editor forgot to enable those direction :)

Thanks for you help you are very helpful
