Besides the point: is this the place to discuss the content of the guidelines/editing rules?

About the topic? Are we looking for a solution, while there is not really a problem?
Are users complaining about the instructions or naming? Are there UR's?
I'm not really in to problemfocused working methods, and i do like working pro active and structured, but, what is confusing about the way exits are indicated now?

The white on blue panels indicate destinations. So if this guideline would be changed, the same logic should be used as for entrance ramps.

My proposal:
Sortie 8 Louvrange > Louvain - Grez-Doiceau
(The > is the direction indicator in BE)

Actually, if I re-read the wiki paragraph, the current version is already close to my proposal. But that is not reflected in the examples in the table.
Exits are named in the official language of the region where they are located (see cityname). If the exit has been numbered, the number must be added to the streetname. Citynames that are mentioned on the road signs, are added to the streetname, separated by a '/'. Road numbers aren't added to the name of the exit
I agree with your last conclusion.
