Those intersections are built like that either because there is no U-Turn allowed there, or U-Turns legality is questionable, or just to simplify the intersection, where Waze would never have to instruct someone to make that U-Turn in the course of normal routing. It's a typical setup for ramps attached to limited access highways.

What the MP indicates is that someone has made a U-Turn there recently. There's no need to make any changes to the map to fix this. Just mark the MP as Solved and move on to the next one.

In the second instance, Waze is just recording the U-turn a bit short of the actual roadway. Another one that doesn't require any changes to the map. Just mark that one solved as well.

Many of the automatically generated MPs are a "better safe than sorry" scenario that don't require any action. Every once in a while, they'll find an actual missing road or junction that is critical to routing, which is justification for their continued existence.
