orbitc is right, that's an issue on your side (browser). Toolbar position is stored in "local storage" of the browser (a kind of cookie), so in your case there must be something that clears this data.

I think "" is a bug of display in elevation list. Try it yourself: set elevation to "" on a segment, save, then reload the editor. The segment you've just edit has elevation set to "ground" again. So for sure ground is the default.
