Standard bug
Steps to reproduce this issue:
When looking at the revision history of a particular place, where a Place Update Request (PUR) was reviewed, the “Updated” line is showing the correct date, but the wrong username.
In the screenshot, I have added red and purple lines to show where it seems to be taking each part from. Somehow it’s combining the date of the most recent edit (the PUR review) with the username of the editor from 2 edits ago.

I made some edits after taking that screenshot, which means it doesn’t show the bug anymore, but perhaps the revision history will help the devs figure out why it happened in the first place.

I’d like to let you know that a fix has been rolled out addressing this edit history mismatch. Moving forward, the “last updated” date and editor name should display correctly once a PUR is accepted or rejected.
While the team is still working on a few remaining edge cases (and older, existing records cannot be updated retroactively), this primary issue should now be resolved.
Could you please check this when you come across new accepted or rejected PURs and verify if the details update properly on your end? Let me know if you run into any further discrepancies.
