No. In plenty of situations where freeways/highways meet, ramps are not necessary, mostly because they look odd. Case in point, see this interchange: https://www.waze.com/beta_editor/?zoom=4&lat=45.36938&lon=-122.75758&layers=BFTFFTTTTTTTFTTTTTFTTTFTT, looks goofy visually with ramps connecting. This is done especially when the ramp is start or a continuation of ONLY a highway or freeway.
