From 9E: Same.
From Middle Rd S: Right onto 9E allowed.
From Middle Rd N: Right onto 9W allowed.
Straight across 9 on Middle Rd not allowed in either direction.

Almost right. Uturns on rt 9 are allowed in both directions at that intersection (there are widened uturn lanes specifically to accommodate the uturners).
Everything else looks right.
